An elemental diet is a liquid formula that delivers essential nutrients in their most basic “elemental” forms.
Elemental shakes contain complete nutrition for a limited period, providing all necessary nutrients without the extensive digestive effort that food requires. This allows the digestive system to heal and recover.
Specifically in cases of Small Intestinal Bacterial Overgrowth (SIBO), the elemental diet works by depriving overgrown bacteria in the small intestine of their food source, effectively eliminating them and helping to “reset” the microbiome.
How exactly does this work?
The elemental diet offers many benefits to those suffering with SIBO.
Anti-Bacterial Properties
Elemental shakes are absorbed very rapidly and within the first few feet of the small bowel depriving bacteria of any food. After a few days without a food source, the overgrown bacteria begin to starve and die off. This allows bacteria to be eradicated without the use of any antibiotics or herbs.
Anti-Inflammatory Benefits
An elemental diet harnesses the anti-inflammatory benefits of bowel rest while still feeding and nourishing the person. Many of the amino acids found in elemental shakes decrease inflammatory cytokines and increase anti-inflammatory cytokines. The low residue, hypoallergenic ingredients can decrease GI irritation and calm overactive immune reactions.
Easily Absorbed Nutrients
Having SIBO can impair digestion and nutrient absorption. The elemental diet contains easily absorbed, hypoallergenic nutrients which ensure adequate nutrition for those suffering with SIBO.
How effective is an elemental diet?
In 2004, Dr. Mark Pimentel conducted one of the earliest studies on the use of an elemental diet for hydrogen-dominant SIBO. He found that a 2–3 week course was approximately 80–85% effective in eliminating bacterial overgrowth.
Nearly two decades later, in 2023, Dr. Ali Rezaie expanded this research by evaluating the elemental diet for both hydrogen SIBO and methane IMO. After just 2 weeks of treatment, the study demonstrated a 100% eradication rate for hydrogen SIBO, an 83% overall success rate for resolving SIBO/IMO (based on repeat lactulose breath testing), and an 82% improvement in gastrointestinal symptoms.
This is considered one of the most effective treatment options available for SIBO, particularly in more difficult or relapsing cases, and often outperforms both antibiotics and herbal protocols.

Does the elemental diet work for hydrogen, methane & hydrogen sulfide?
While the earliest elemental diet research focused primarily on hydrogen-dominant SIBO, clinical experience and newer research suggest that elemental diets can be highly effective for all three gas types: hydrogen, methane, and hydrogen sulfide.
Hydrogen SIBO
Hydrogen SIBO tends to respond most predictably because the elemental diet rapidly removes the fermentable carbohydrates that feed bacterial overgrowth. Since elemental shakes are absorbed very quickly in the upper part of the small intestine, very little is left behind for bacteria to ferment. Without a steady food source, the overgrown bacteria begin to starve and die off.
This is one reason hydrogen-dominant SIBO often shows the clearest and fastest response to an elemental diet. As bacterial overgrowth decreases, symptoms like bloating, gas, abdominal discomfort, and loose stools often begin to improve as well. For many patients, the elemental diet acts almost like a “reset,” helping reduce bacterial burden while giving the digestive system time to rest and heal.
Methanogen Overgrowth (IMO)
Methane overgrowth (now called Intestinal Methanogen Overgrowth or IMO) is produced by archaea rather than bacteria. Unlike hydrogen-dominant SIBO, methane overgrowth involves methanogens, organisms that feed on hydrogen gas produced by other microbes in the gut.
This relationship is important because methanogens cannot produce methane on their own; they depend on hydrogen as their fuel source. In other words, hydrogen often drives methane production. When an elemental diet removes the fermentable carbohydrates that feed bacterial overgrowth, hydrogen production drops significantly. As hydrogen levels fall, methanogens lose their primary fuel source, which helps reduce methane as well. This is why the elemental diet can be highly effective for methane IMO, even though these cases sometimes require a longer course or additional support.
Hydrogen Sulfide (ISO)
Hydrogen sulfide SIBO (now referred to as Intestinal Sulfide Overgrowth, or ISO) can be more complex, especially in patients with significant food sensitivities, diarrhea, histamine issues, or sulfur intolerance. Many of these patients feel reactive to a wide range of foods and supplements, which can make treatment feel especially frustrating. In these cases, the low-residue, hypoallergenic nature of an elemental diet can be helpful by reducing digestive burden while still supporting adequate nutrition.
Most elemental formulas contain sulfur-containing amino acids like methionine and cysteine, which raises understandable concern about whether they could worsen symptoms. In practice, elemental diets usually still work well because these nutrients are absorbed very quickly in the upper small intestine, leaving very little behind to feed microbes further down. The bigger drivers of hydrogen sulfide overgrowth are often fermentation, poor motility, and microbial imbalance rather than sulfur amino acids alone, though very sensitive individuals may benefit from a different approach.
Clinically, we have found that success depends less on the gas type itself and more on proper preparation, appropriate formula selection, and having a clear plan for both treatment and prevention of relapse afterwards.
Interested in an elemental diet?
If you have been diagnosed with SIBO and believe that an elemental diet is the appropriate approach for you, please see the information on our self-guided program, the Elemental Diet Success Plan (EDSP). The EDSP has provided hundreds of participants with the exact roadmap that we have been using in our clinical practice with outstanding results.
For those who would benefit from additional support through their elemental diet process, I also offer a program that combines 1:1 Elemental Diet Guidance alongside the EDSP program.
